✅ CDL PERMIT CHECKLIST: WHAT TO BRING
- Valid Driver’s License
Your current state-issued license is required to apply. - Proof of Identity
Typically a birth certificate, U.S. passport, or permanent resident card. - Proof of Social Security Number
Social Security card, W-2 form, or pay stub with full SSN. - Proof of U.S. Citizenship or Lawful Presence
Required if not covered by the identity documents above. - Proof of State Residency
Utility bill, lease agreement, or official mail with your name and address. - Medical Examiner’s Certificate (DOT Physical)
Must be completed by a certified medical examiner and current (within 24 months). - Payment for Fees
Permit fees vary by state—expect to pay $25–$50 depending on your location. - Proof of ELDT Theory Training (if required by your state)
